It could be argued that big is not always best. This is the philosophy to which we hold as a practice. We are one of the newer training practices in Wigan and consequently still full of enthusiasm for the challenges of the Registrar year. Our registrar is a genuine part of the practice team. He or she has a real input on not only the patient's care but also how the practice as a whole develops. Changes that they suggest are actually listened to and introduced where appropriate.
We genuinely feel we know and understand most of our patients. We are small enough to still retain much of the virtues of some old fashioned general practice. Many of our patients are related and we have a number of families represented through the various generations. We are privileged to be working in a deprived area of town.
As a result there is a large amount of genuine pathology in both a physical and psychological sense experienced in many of our patients. However, the patient population is not so numerous as to over whelm the inexperienced by its excessive number.
However we are also modern enough to understand that computerisation is vital for modern health care management. We have for a number of years been paper light and ran disease registers electronically in all the areas defined by the New GMS Contract. This has allowed us to construct excellent record systems where the patient complexities have largely been teased out to make understanding of their health needs and concerns slightly easier. In addition our small, dedicated team of clinicians and staff, through their personal knowledge of the patients, are often able to help clarify subtle nuances of their behaviour.
We also accept that registrars are here to learn in a genuine adult manner in response to personally perceived weakness. This is after all what we do. Personal development should be focused and learner centred. In addition it also should be planned strategically and be verifiable as having achieved its objectives. We hope that you will find at our surgery it is all of this.
We have the necessary video equipment and expertise to help registrars develop their consultation techniques, especially if they were thinking of trying to pursue higher examinations such as the MRCGP. We have an excellent success rate in this area will all registrars to date passing successfully. In addition the practice has a library that would allow personal study to undertaken in the relevant areas. All consultation rooms have direct Internet access to allow on-line searching for particular clinical answers. In addition we routinely use audit to monitor and develop our patient care programmes and so many of the rigours of Summative Assessment with our help should present little challenge to our registrar.
We are proud to have recently moved into new LIFT premises. This "superhealth centre" contains two other GP training practices and two undergraduate training practices. It has a purpose built learning suite for all of Wigan within it allowing easy access to a number of unique facilities. In addition the rest of the extended Primary Health care team such as district nurses, health visitors, chiropodists, physiotherapists and phlebotomists are now literally at the end of the corridor. The potential learning experience for a Registrar of working in such a facility for twelve months and gaining an insight into the wider aspects of team working will we suspect reap dividends in their future career.
If someone is hoping for a year as an inconsequential non functioning cog in a huge team then this may not be the right post for you. Although we have a full primary health care team because we work closely together we actually know each other. We listen to each other and respect each other. We therefore actually do work effectively as a team in a true sense.
